Align care with personal goals and avoid the ER
– Welcome! This site was originally created in 2014 as a place to stash favorite resources for my own work as a nurse practitioner providing house calls for seniors in the Metro Detroit area.
– Over time, it has become useful to my coworkers, students, patients and their caregivers.
– Most items focus on advance care planning and palliative care. It can also be useful to primary care physicians, home care nurses, discharge planners and anyone else with a goal to help people age well in place.
My hope is to strengthen partnerships between complex patients and their primary care physician, and help both make the most of neighborhood care services to stay home and reduce time spent in the hospital.
– This is something I use personally for my own practice to provide resources and education. It is shared freely in case it will help others, and it is never medical advice.
– The website comes with an automatic prompt to accept cookies, but I do not use or benefit from them. There will never be advertisements and I do not collect or use visitor data.
